Make sure that top-level css is not inlined twice. this is probably not a problem for link-rel elements inside <template> tags, but for <link-rels> that occur on the top of several imported files we are probably duplicating things.

It might be nice to also warn if a specific css file is inlined more than once for any given entry point. We could either warn and suggest that they change the option in their pubspec to not inline that file, or we could automatically not inline it.
